and any other information you are going to verify. |

Complete Request for Tenancy Approval (RFTA). |

1. Make sure you include Tax Identification Number. |

2. Include a copy of your Recorded Grant Deed. |
|
Rental unit must pass Housing Quality Standards
Inspection. |
|
1. Up to 14 days after RFTA is received by SHRA. |
|
2. Tenant cannot be authorized to move in until unit
passes. (Unless the Tenant was already occupying the unit at the time they
received their voucher) |
|
3. Additional inspection if unit does not pass. |
|
Housing Assistance Payments Contract (HAP) and
Tenancy Addendum will be mailed to you. |
|
1. Please allow 7-14 days after inspection. |
|
2. HAP Contract is for one year, then it is month to
month. |
|
HAP Contract and Tenancy Addendum must be signed
by Owner/Agent and Tenant and returned to SHRA. |
|
1. Include a copy of your lease. |
|
2. SHRA will mail back copies for Owner/Agent and tenant |
|
Payment to Owner/Agent will be mailed.
Please allow 7-14 days from the time SHRA receives the HAP Contract and
Tenancy Addendum. |
