To File A Claim:

Filing a claim with the Agency is a process regulated by state law. The Agency cannot give you legal advice regarding the validity or worth of a claim. Most people generally will consult with an attorney not connected to the Agency for assistance in filing a claim. All claims must be filed in a timely manner. Delay in filing a claim may result in a loss of your rights.

If you wish to file a claim, please print and complete the following form and deliver it to the Agency’s address:

SHRA, Legal Department, 630 I Street, Third Floor, Sacramento, CA 95814. You may use your own form, but it must comply with applicable law. Please do not attempt to return the form provided below by e-mail.

To Make A Public Records Request:

If you wish to make a request for information under the California Public Records Act, we must receive your request in writing. Please deliver your request to: SHRA, Legal Department, 630 I Street, Third Floor, Sacramento, CA 95814.

In order for us to assist you, please make your request as specific as possible. General or very broad requests tend to produce a large number of documents and could delay your request. Please remember that the Agency is not required to create any new documents, only provide records upon request that are already held by the Agency. For example, if you request a list of names and that list does not exist, we will simply notify you that no such record exists.

Within 10 days of the receipt of your request, you will be notified when the requested records will be available for you to review in our offices. Copies of any records reviewed can be made at a charge of ten cents per page.
